Driving Licence Eyesight Report Form (D502)

All first time applicants for a drivers licence learner permit must submit a Driving Licence Eyesight Report. This report can be completed by a medical practitioner or a registered opthalmic optician.

Driving Licence Eyesight Reports are also required is the applicant previously wore glasses/ lenses but are no longer required to do so. Sometimes it is also needed if the applicant is exchanging their licence from a non EU country.  

The report must be signed by the applicant in the presence of the doctor or optician and then submitted within one month of the date of application at an National Driving Licence Service centre.
